When the Patient Suffers: Optimization of Transitions between Care Facilities

    • SHELLY DUBIN, MSN,GNP-C, has been a geriatric nurse practitioner at New York Presbyterian Hospital at the Allen Pavilion, Division of Geriatric Medicine and Aging. Her practice is focused on the care of the frail nursing home patient. In this role, she has been able to facilitate transitions of care between the nursing home and the acute care facility. Shelly works with the medical house staff consulting on geriatric syndromes. She has coordinated workshops for nursing staff to enhance their knowledge of the older patient and has been a preceptor for nurse practitioner students from Columbia School of Nursing and New York University School of Nursing.
